An application form can be gotten and submitted - from Ms. Uma Maheshwari  who sits in the basement of Sobha Quartz

Submit the following with the appln form :

- 1 passport photo
- address proof
- a photo id proof ( driving license, ration card, voter id card pr passport copy)
- PAN # ( if you have one, otherwise is OK)
